Requirments

The following is required to register for the sacrament of confrimation:

Registration Form/ Memorializaton Form

Emergency Medical Release Form

Current Immunization Record

• Registration fee- $200.00 due at time of registration(make checks payable to Saint Didacus) Fee is Non-refundable
• Evidence that family is registered in the parish(Sunday donation envelope)
• Baptismal Cerificate*
• First Communion Certificate*
• Candidate must be present at time of registration
  

Spaces will be filled on the First come, First served basis. There are limited number of spaces available and spaces do fill up fast, so be sure to register early. Your son or Daughter is not considered registered until all of the specified items have been received and the parent and candidate have met with the confirmation Coordinator for an initial interview

El siguiente información es necesaria para registrarse en el programa del sacramento de la confrimation:

• Formulario de Inscripción / Autorización con foto
• Formulario de Autorización de Emergencia Medica
• Registro de vacunación actualizada.
• Pago de la registración $ 200.00 al momento de la inscripción (cheque a nombre de St Didacus) Este pago no es reembolsable
• Comprobante que la familia se ha registrado en la parroquia (sobre donación dominical)
• Certificado de Bautismo *
• Certificado de primera comunión*
• El candidato debe estar presente en el momento de la inscripción

*Nota :Necesitamos certificado aun si el candidato ha completado estos Sacarments en la iglesia de Saint Didacus

Los espacios serán llenados conforme van llegando. Hay un número limitado de espacios disponibles y se llenan rápido, así que asegúrese de registrarse con anticipación. Su hijo o hija no se considera registrado hasta que todos los requisitos se han cumplido y el padre/madre y el candidato se han reunido con el Coordinador de confirmación para la entrevista inicial.
